D-chiro-inositol (DCI) is one of nine inositol stereoisomers. It is a key component of inositolphosphoglycan (IPG) mediators that are second messengers for insulin action. Women with PCOS have reduced DCI-IPG release in response to insulin, contributing to insulin resistance.
DCI is incorporated into IPG mediators that activate pyruvate dehydrogenase phosphatase downstream of the insulin receptor, facilitating glucose uptake and glycogen synthesis. In PCOS, DCI supplementation (600 mg/day) restores ovulation, reduces free testosterone, and improves insulin sensitivity. The 40:1 myo-inositol:DCI ratio mimics the physiological ratio in human follicular fluid.
